A regular bottom job should be a three thousand or so. I wouldn't do the other stuff unless there is a known problem.
For chain plates, we had a local FL company make replicas of ours. In the end it was about $1600.00 for all of them, including the bow and stern one. The pricey part would be having someone remove them and reinstall them, especially because it is time consuming to remove the cabinetry in the head to get to the back of the port side ones.
It's not that hard though. I did it when I was totally green and not very knowledgeable. I would recommend doing it that way. Pay to have them made, but do the work yourself.
